Must, don't have to, should: ต้อง, ไม่ต้อง, ควร

How it works
1 Must, and no need to
ต้องdtâwng + verb means 'must, have to, need to': ต้องกินยาdtâwng gin yaa (you have to take medicine), ต้องไปหาหมอdtâwng bpai hǎa mǎw (I need to see a doctor).
ไม่ต้องmâi dtâwng + verb means 'you don't have to, no need': ไม่ต้องกลับมาmâi dtâwng glàp maa (no need to come back). It never means 'mustn't': for advice against something, use ไม่ควรmâi kuan (below).
- ต้อง dtâwngกิน ginยา yaa
You have to take medicine.
- ต้อง dtâwngไป bpaiหา hǎaหมอ mǎw
I need to see a doctor.
- ไม่ต้อง mâi dtâwngกลับ glàpมา maa
You don't need to come back.
2 Should and shouldn't
ควรkuan + verb gives advice, softer than a rule: ควรพักผ่อนkuan pák-pàwn (you should rest). Put ไม่mâi in front for 'shouldn't': ไม่ควรกินเผ็ดmâi kuan gin pèt (you shouldn't eat spicy food).
To ask for advice, add ไหมmǎi: ควรไปหาหมอไหมkuan bpai hǎa mǎw mǎi (Should I see a doctor?).
- ควร kuanพักผ่อน pák-pàwnเยอะๆ yóe-yóe
You should get plenty of rest.
- ไม่ mâiควร kuanกิน ginเผ็ด pèt
You shouldn't eat spicy food.
- ควร kuanไป bpaiหา hǎaหมอ mǎwไหม mǎiครับ kráp
Should I see a doctor?
Men ควรไปหาหมอไหมครับ Women ควรไปหาหมอไหมคะ

3 Saying what's wrong
Four patterns cover most of it:
- ปวดbpùat + body part, a deep ache: ปวดหัวbpùat hǔa (headache), ปวดท้องbpùat táwng (stomach ache)
- เจ็บjèp + body part, a sharp or surface pain: เจ็บคอjèp kaw (sore throat)
- เป็นbpen + illness: เป็นไข้bpen kâi (I have a fever), เป็นหวัดbpen wàt (I have a cold)
- รู้สึกrúu-sùek + how you feel: รู้สึกไม่สบายrúu-sùek mâi sà-baai (I don't feel well), รู้สึกเวียนหัวrúu-sùek wian-hǔa (I feel dizzy)
- ปวด bpùatหัว hǔa
I have a headache.
- เจ็บ jèpคอ kaw
I have a sore throat.
- เป็น bpenหวัด wàt
I have a cold.
- รู้สึก rúu-sùekไม่สบาย mâi sà-baai
I don't feel well.
4 At the doctor's
The doctor asks เป็นอะไรbpen à-rai (what's wrong?) or มีอาการอะไรmii aa-gaan à-rai (what are your symptoms?), and often เป็นมากี่วันแล้วbpen maa gìi wan láew (how many days have you had it?). Say when it started with เริ่มrôem: เริ่มเมื่อวานrôem mûea-waan (it started yesterday).
When it's over, you're หายแล้วhǎai láew (better now), or at least ดีขึ้นแล้วdii kûen láew (getting better).
- เป็น bpenอะไร à-rai
What's wrong?
- เริ่ม rôemเมื่อวาน mûea-waan
It started yesterday.
- หาย hǎaiแล้ว láew
I'm better now.
Tips
- •ไม่ต้องกลับมาmâi dtâwng glàp maa is good news: you don't have to come back. It doesn't mean you're not allowed to!
- •Soften advice to a friend with นะná: พักผ่อนเยอะๆนะpák-pàwn yóe-yóe ná (get lots of rest, okay?).
- •Thais also say ปวดหัวbpùat hǔa about anything that gives them a headache: a broken air-con, a visa form, the traffic.
